Immunization

Children attending Nathanael Greene Academy must show proof of immunization by submitting a Georgia Certificate of Immunization (Form 3231) or Supplemental Vaccine Certificate (Form 3189, for sixth grade). Form 3231 has a designated space for an expiration date to indicate if a child needs additional vaccines for attendance.

All incoming 7th graders and any new entrants into the Georgia school system will be required to have two immunizations, one for tetanus, diphtheria, and pertussis (Tdap) and one for meningitis (meningococcal disease) before starting the new school year. A second dose for meningitis is also required for all 11th graders. Please check with your health care provider or local health department to see if your rising 7th grader has had these immunizations. An updated Certificate of Immunization (Form 3231) will be needed before school starts.
